Reporting to the Senior Director Case Management, the Nurse Coordinator will lead and optimize patient-centered care coordination across the continuum by overseeing utilization review, discharge planning, and transitions of care in an acute-care hospital setting. Daily responsibilities include facilitating interdisciplinary rounds, developing individualized care plans, securing prior authorizations, collaborating with physicians, social work, and post-acute providers, arranging home health or skilled nursing placements, educating patients and families on care pathways and community resources, monitoring length of stay and avoidable days, reducing readmissions, ensuring accurate and timely documentation in the EHR, and maintaining compliance with CMS, payer, and Joint Commission standards. This role mentors RN Case Managers, analyzes throughput and quality metrics, escalates barriers to care, drives process improvement, and serves as a resource for complex cases.
