Primary Care Physician (PCP)

Salary and Benefits

• Base salary of $250,000 for new grads, up to $280,000 for experienced PCPs with value-based care experience
• Earn over $320,000 in year one with incentives
• Annual incentive bonus up to 28% of base salary (average ~20%)
• Partnership opportunity after 3 years (no buy-in required)
• Sign-on bonus up to $15,000
• 25 days of paid time off (PTO)

Responsibilities

• Work Monday through Friday from 8:00 AM to 5:00 PM
• Provide outpatient primary care to a senior population (ages 65+)
• Practice in a value-based care model with a patient-first focus
• Manage a panel size of up to 400 patients
• See an average of 10–13 patients per day
• Shared call schedule: 1 week every 8 weeks, 3–5 total calls per week on average
• Complete charting during work hours
• Collaborate with a dedicated care team, including an MA, scheduler, referral specialist, nurse, case manager, social worker, and outreach specialist
• Engage with community-based wellness programs such as cooking classes, yoga, and Silver Sneakers
• Utilize in-house labs, EKGs, and echocardiograms

Requirements

• MD or DO degree
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Must be computer proficient and have no disciplinary or legal actions (e.g., Medicare fraud, sexual misconduct, anti-kickback violations)
• Will consider 2026 residents
• Commitment to patient-centered care in a concierge-style model
